Speedometer Bouncing: Cable, Adapter, or Something Else?

My bud has a 1997 Toyota T100 truck, manual transmission, with a bouncing speedometer. The speedometer's MPH also reads about 10% higher than the actual MPH, based on comparisons with road caution speed meters. We figure it's either the cable, elbow adapter, needs a lube, or something else. I am going to get under the T100 sometime this week for a good look. Requests for help:

-- Anyone know of a do-it-yourself web site or video for replacing the cable?
-- Is the connection to the tranny case on the driver's side, front side of the tranny case? Please describe the location.
-- What lubricant do people like for this?
