2003 Tacoma Transmission

My wife's 2003 Tacoma (4cyl., automatic, regular cab) has about 150,000 miles on it. Yesterday very suddenly it started producing a very heavy CLUNK when shifting from first to second. Not a little clunk, a big one that can't be ignored. Otherwise it shifts and runs normally. The trans fluid looks good. Any insights would be welcome. Thanks.

I'm not a transmission expert by any means. With that said, "the trans fluid look good" meaning it's not low on fluid? Smells burnt or looks cooked?

Any idea if any previous service was done to transmission?

The A340 has always been a solid performing transmission, very few issues.
I've honestly never heard of one having an issue your describing, even with high miles. Your 150k isn't high miles for this drive train.
One thing I would check is trans line pressures, high line pressures will cause rough shifts and can cause clunking noises. Very possible theres a blocked filter

Might be time to find a really good transmission mechanic. What you described doesn't sound good.
